Airmenselection Consent Form – Everybody should be able to make informed choices about their medical care. Medical treatments can be invasive, so patients should be able, in the end, to decide from the facts about risks of their body, how it will be treated. Informed consent constitutes a lawful condition that requires that a patient be given a complete and accurate description of his or her physical health and the treatment suggested by the physician in charge. Decision Making Capacity

In other circumstances, patients may not be able communicate their choices to health professionals. If this happens, the patient is said not to have adequate capacity to make decisions. If a family member is not present, or court-appointed representative can perform informed consent instead.
Patients that are strongly influenced by their emotions – anxiety or fear, for instance are deemed lacking the ability to make decisions. Patients who are in the state of unconscious cannot make decisions on their own, and outside parties need to consent to treatment instead.
Items in an Airmenselection Consent Form
There are certain elements that are common to all consent forms:
The patient’s medical conditions/diagnosis
The treatment recommended by the acting physician
The risks and benefits associated with this treatment
Alternative treatments that are offered, as are their benefits and risks
The risks and benefits associated with accepting no treatment at all
These details must not only be recorded in the documentation however, they must be discussed with the patient. In this way, he or will be able to comprehend all the details of the scenario and receive direct responses to any concerns that might arise.
