Amputation Consent Form – Every person should be able to make informed decisions about their health. Decision Making Capacity

In other situations patients may not be able to effectively communicate their decision to health professionals. Under these circumstances the patient is said not to have adequate capacity to make decisions. If a family member is not present, or court-appointed representative could then be able to take over informed consent.
Patients that are strongly influenced by their emotions – such as anxiety or fear, as an example are deemed not able to make decisions. People who are not conscious cannot make decisions on their alone, and external parties have to give consent for treatment instead.
Items in an Amputation Consent Form
Certain elements are common to all consent forms:
The diagnosis or medical condition of the patient.
The recommended treatment is suggested by the acting physician
The risks and benefits that come with this procedure
Alternative treatments that are offered, as are their potential risks and benefits
The risks and benefits that come with accepting no treatment whatsoever
