There are certain elements that are commonly included in informed consent forms:

The diagnosis or medical condition of the patient.

The treatment that is recommended by the physician in charge

The risks and benefits that come with this method of treatment

Alternative treatments are readily offered, as are their risks and benefits

The risks and benefits that come with not accepting any treatment whatsoever

These items must not only be documented But they also need to communicated with the person receiving the treatment. This way, he can be fully aware of the details of the situation and will be able to get immediate answers to any concerns that might arise.
