In some instances, patients may not be able to effectively communicate their decision to health professionals. In such situations it is believed that the patient to lack the necessary capacity for decision-making. The family member, or court-appointed representative could then be able to provide informed consent instead.
Patients who are greatly influenced by their emotions, like anxiety or fear, for instance they could be judged as not having the capacity to make decisions. Patients who are in the state of unconscious cannot make decisions on their independent of themselves, so outsiders must provide consent for treatment instead.
Items in an Free Consent Form Template
There are certain elements that are generally included in informed consent forms:
The patient’s medical condition/diagnosis
The treatment recommended by the doctor in charge
The risks and benefits that come with this procedure
Alternative treatments that are available, along with their risks and benefits
The benefits and risks associated with refusing treatment at all
Not only must these items be recorded in the documentation however, they must discuss the situation with patients. So, he can fully comprehend what is happening and can get direct answers to any issues that may arise.
