Decision Making Capacity

An individual from the family or court appointed representative in this case, can take over informed consent.
Patients that are strongly influenced by their emotions – such as anxiety or fear, for example could be classified as not having the capacity to make decisions. Those who are unconscious clearly can’t make decisions on own, and outside parties have to give consent for treatment instead.
Items in an Interview Consent Form Template
There are certain elements that are included on all informed consent forms:
The patient’s medical diagnosis/condition
The treatment that is recommended by the acting physician
The risks and advantages associated with this treatment
Alternative treatments are also available, along with their risks and benefits
The potential risks and rewards with not accepting any treatment whatsoever
These details must not only be recorded in the documentation However, they should also communicated with the person receiving the treatment. In this way, he or she will fully understand the specifics of the situation and will be able to get immediate answers to any questions that may arise.
