Items in an Irb Consent Form Template
There are certain elements that are included on all informed consent forms:
The patient’s medical condition/diagnosis
The procedure recommended by the physician who is acting
The risks and advantages associated with this method of treatment
Alternative treatments that are offered, as are their risks and benefits
The risks and benefits associated with accepting no treatment at all
The items should not only be recorded in the patient’s medical records however, they must been discussed by the patient. So, he will be able to comprehend the particulars of the case and will be able to get immediate answers to any questions that have arisen.
