Key Informant Interview Consent Form – Everybody should be able to make educated decisions about their health. Decision Making Capacity
In certain instances the patients aren’t equipped with the capabilities to fully understand their options in terms of treatment and the risks/benefits associated with each one. In other instances patients may not be able to explain their decisions to health workers. Under these circumstances the patient is said not to possess the proper decision making capacity. A family member or court appointed representative in this case, can perform informed consent instead.
Patients who are heavily influenced by their emotions – such as anxiety or fear, for instance – may be determined as not having the capacity to make decisions. The patients who are unconscious are unable to make decisions on their own, and outside parties have to give consent for treatment instead.
